Causes of Hip Pain
Pain can arise from the structures that are within the hip joint or from the structures surrounding the hip joint. The most important thing is to ask the patient to locate the site of pain. ask the patient to point at the site of pain. When the patient states that their hip hurts, it doesn’t mean that the pain is coming from the hip joint itself, so ask the patient to point at the site of the pain. The pain can arise from structures that are within the hip joint or from structures surrounding the hip. The hip joint is a weight bearing joint. The joint consists of two main parts: femoral head (ball) and acetabulum (socket). The hip pain can be anterior hip pain (deep groin pain). The pain can be lateral hip pain. the pain can be posterior hip pain. The pain can be far posterior hip pain, coming from the sacroiliac joint and the lower spine. Anterior hip pain is usually deep within the groin, and it can result due to arthritis of the hip. Conservative treatment is physical therapy, anti-inflammatory medication, possible injections, and surgery is done in late cases, usually by total hip replacement. It is usually diagnosed by clinical examination with a provocative test of flexion, adduction, and internal rotation. The diagnosis is confirmed by an MRI arthrogram. Conservative treatment is therapy, anti-inflammatory medication, and injections. Surgical treatment provides good result and is usually done by arthroscopic debridement or repair of the tear. Stress fracture is usually diagnosed by an MRI. The x-ray may be normal. Early diagnosis is important before the fracture displaces and gives a bad result. Treatment of avascular necrosis is usually surgical fixation of the fracture. Fixation of the fracture is usually performed utilizing screws. Femoral head replacement is done in rare, late cases. Avascular necrosis means death of a segment of the bone. When the blood supply of the femoral head is interrupted, a segment of the bone dies and becomes necrotic (femoral head will collapse). Treatment for early stages of AVN without collapse of the femoral head include decompression by drilling of this segment in the femoral head to bring a new blood supply to the area. Vascularized fibular graft may be used also. In severe cases with collapse of the femoral head (usually diagnosed by an x-ray), the treatment is usually total hp replacement. Treatment of an inflamed bursa is usually conservative treatment of physical therapy, anti-inflammatory medication, and injection. Surgical treatment by excision of the bursa is rarely done. In case of chronic, resilient trochanteric bursitis, try to get an MRI to exclude a tear of the abductor muscles of the hip (gluteus medius and gluteus minimus muscle tear). Posterior hip pain is usually due to piriformis syndrome. The sciatic nerve can be irritated from the piriformis syndrome. Treatment is usually physical therapy, stretching, anti-inflammatory medications, and injections. Surgical treatment is usually rare. It is the last resort. It includes release of the piriformis tendon and exploration of the sciatic nerve. It is done in cases that fail to improve with conservative treatment. Far posterior pain may come from the sacroiliac joint or from the lower spine conditions. Sacroiliac joint (SI) problems is a challenging diagnostic and treatment entity. There are a lot of clinical diagnostic examinations that can be used to diagnose sacroiliac joint (SI) problems such as the Faber test and others. However, injection of the SI joint is probably the method to diagnose pain originating from the SI joint. If there is improvement of the condition of the patient after injection of the SI Joint, then we will probably consider that the problem is in the SI joint. The SI joint problems are usually under estimated and are unappreciated. Lower spine conditions can cause referred pain to the buttock and hip area. In fact, symptoms of hip and lower spine conditions can overlap or both of them can coexist in the same patient. You have to separate pain from the hip from pain that comes from the spine.

Common reasons include muscle imbalance whether it is weakness or tightness, poor alignment ok knee cap, over use of leg resulting from excessive impact of high activities, flat foot deformity and finally the anatomical abnormalities in the knee cap shape and the grove in which it sits. Collectively all these conditions result to a condition known as chondromalacia patella which results in pain and inflammation. This is typically seen in young adults particularly women, they typically present with the pain in the front of the knee which is precipitated by going up or down stairs or when they try to sit up and walk after a prolonged period of rest.

Lateral Hip or Trochanteric Bursitis – Causes and Treatment

Trochanteric bursitis causes pain on the side of the hip, but it can also refer pain down the side of the leg or into the back. It’s a bit of a tricky condition to diagnose, as there are several other injuries that can cause very similar pain. Dyspareunia is pain with intercourse or pain post intercourse. Pain with intercourse itself can have a strong emotional impact. Quite often, it can lead to avoidance of intercourse with your partner. It can lead to relationship issues with your partner. It can lead to a fear of intercourse and this fear can lead to something we call Vaginismus. When patients are in fear of intercourse, the muscles surrounding the vaginal opening can clamp down, so to speak. It can also lead to emotional distress, anxiety, as well as sleep disturbances, all secondary to the pain with intercourse. We do always ask, is it more of a superficial burning sensation? Which can often be associated with entrance pain or Vulvodynia symptoms, and at that point, it’s important to evaluate any underlying hormonal cause or underlying infection such as a yeast infection. Or is it more of a deep, penetrating pain? The pelvic floor muscles are important in evaluating and treating as well as any underlying organ disorders potentially within the female system such as ovarian cyst or a fibroid.

Sex After Joint Replacement Surgery is a topic that makes people uncomfortable, but it’s a important to talk about. You can expect a period of time after surgery where you’re not going to be physically able to have sex. After this period, there’s going to be a period where you can but you just have to be cautious. Use a common sense approach with sexual activity.

There’s also a difference between positions for hips and knees. For hips, there’s a few differences. First, depending on the approach your surgeon used, there are different precautions. Whether your surgery was done from the front or the back, will dictate which positions are safe and which positions are unsafe. There’s a lot of literature and articles about what you can and cannot do after surgery and as you recovery.

In terms of knees, the biggest issue is anterior knee pain and range of motion. Often times in the first few weeks after surgery we’re focused on increasing range of motion. Certain positions could potentially flex your knee to the point where it causes discomfort. Three to four weeks after surgery or when you’re able to walk on your own and your pain is controlled off narcotics, is a good time to start experiementing.

Hip Pain at Night: Causes and solutions for lateral hip pain and groin pain lying on your side

If your hip hurts while you’re trying to sleep on your side, physical therapist Christine Koth explains what could be causing hip pain at night and how to adjust your position to get some sleep.

If you have outer hip pain, such as hip bursitis, you may feel pain either on the hip you’re laying on, or the hip you’re not laying on.

If you have pain in the hip you’re laying on, pain caused by direct pressure on the joint, then pay attention to your mattress surface. You may find relief by adding a memory foam pad or mattress topper to your mattress to help distribute your weight more evenly and provide extra cushion for that hip joint.

If you have pain on the hip you’re not laying on, try sleeping with a pillow between your knees and ankles. It may take some time to find a position that works with your pillow, and you may even have to try out pillows of different densities. The goal is to have your legs rest parallel to each other, so your knee is not lower than your hip. This can be accomplished by using a fat pillow, resting lengthwise from knees to ankles.

Sleeping with a pillow between your knees and ankles is a good way to relieve SI joint pain while sleeping on your side, as it reduces the strain on muscles attaching to the top of the thigh bone (greater trochanter). This strain on the hip is directly related to tight hip flexors (specifically a tight iliacus muscle), which causes an anterior pelvic rotation, and pulls on the lower back and hip joint.

Whatever the pillow position you choose, the main purpose of sleeping with the pillow is to alleviate the pull on your hip’s soft tissues and keep your pelvis in a natural alignment.

When you allow your leg to rest naturally while lying down, there is a slight dip that enables your legs to connect and rest. When this happens, your leg’s natural pull adds more pressure to the muscles around your hip joint, which can then cause hip pain when sleeping.

Leg pains and cramps are usually common 4 weeks after caesarian section. The various reasons can be during pregnancy there is loosening of hip joints and ligaments affecting the legs, knees and back but after delivery the body has to come back to normal and this may be painful. So take regular calcium, Vitamin D, Vitamin B12 , iron. Sleep properly, do gentle exercises, stretching exercises, yoga, pranayama, drink plenty of water if possible attend the post natal exercise class. Avoid wearing high heels. Avoid carrying heavy weight. Expose yourself to adequate sunlight for the daily sunshine Vitamin D. rule out other causes by taking a visit to your obstetrician who may also take the help of orthopedician or neurologist to rule out other causes like deep vein thrombosis, hematoma in the epidural site region, sciatica, that is the disk compression, presence of any minor fractures, varicose veins.

Diarrhea, also spelled diarrhoea, is the condition of having at least three loose or liquid bowel movements each day. It often lasts for a few days and can result in dehydration due to fluid loss. Signs of dehydration often begin with loss of the normal stretchiness of the skin and irritable behaviour. This can progress to decreased urination, loss of skin color, a fast heart rate, and a decrease in responsiveness as it becomes more severe. Loose but non-watery stools in babies who are exclusively breastfed, however, are normal.

The most common cause is an infection of the intestines due to either a virus, bacteria, or parasite – a condition also known as gastroenteritis.[2] These infections are often acquired from food or water that has been contaminated by feces, or directly from another person who is infected. The three types of diarrhea are: short duration watery diarrhea, short duration bloody diarrhea, and persistent diarrhea (lasting more than two weeks).The short duration watery diarrhea may be due to an infection by cholera, although this is rare in the developed world. If blood is present it is also known as dysentery. A number of non-infectious causes can result in diarrhea. These include lactose intolerance, irritable bowel syndrome, non-celiac gluten sensitivity, celiac disease, inflammatory bowel disease, hyperthyroidism, bile acid diarrhea, and a number of medications.In most cases, stool cultures to confirm the exact cause are not required.
