Dyspareunia is pain with intercourse or pain post intercourse. Pain with intercourse itself can have a strong emotional impact. Quite often, it can lead to avoidance of intercourse with your partner. It can lead to relationship issues with your partner. It can lead to a fear of intercourse and this fear can lead to something we call Vaginismus. When patients are in fear of intercourse, the muscles surrounding the vaginal opening can clamp down, so to speak. It can also lead to emotional distress, anxiety, as well as sleep disturbances, all secondary to the pain with intercourse. We do always ask, is it more of a superficial burning sensation? Which can often be associated with entrance pain or Vulvodynia symptoms, and at that point, it’s important to evaluate any underlying hormonal cause or underlying infection such as a yeast infection. Or is it more of a deep, penetrating pain? The pelvic floor muscles are important in evaluating and treating as well as any underlying organ disorders potentially within the female system such as ovarian cyst or a fibroid. Video Rating: / 5
Sex After Joint Replacement Surgery is a topic that makes people uncomfortable, but it’s a important to talk about. You can expect a period of time after surgery where you’re not going to be physically able to have sex. After this period, there’s going to be a period where you can but you just have to be cautious. Use a common sense approach with sexual activity.
There’s also a difference between positions for hips and knees. For hips, there’s a few differences. First, depending on the approach your surgeon used, there are different precautions. Whether your surgery was done from the front or the back, will dictate which positions are safe and which positions are unsafe. There’s a lot of literature and articles about what you can and cannot do after surgery and as you recovery.
In terms of knees, the biggest issue is anterior knee pain and range of motion. Often times in the first few weeks after surgery we’re focused on increasing range of motion. Certain positions could potentially flex your knee to the point where it causes discomfort. Three to four weeks after surgery or when you’re able to walk on your own and your pain is controlled off narcotics, is a good time to start experiementing.

.Over the years I have treated dozens and dozens of people seeking help with their squat. A particularly common issue people report is pinching hip pain in their hip/groin region at the bottom of the squat position. There are a handful of orthopedic diagnoses people with this issue get handed/labeled to explain what may be going on:
1. Labrum pathology
2.Femoral Acetabular Impingement (FAI)
3. Hip Flexor Tendinitis
4. Hip Osteoarthritis

🚨PSA🚨these labels more often than not hinder, not help people get back to squatting. For too often I find myself educating people in the clinic that it is OK TO SQUAT, we just need to analyze what is going on and make MODIFICATIONS if needed.

In this post, I point out two of the MOST COMMON REASONS why people experience pinching symptoms with squatting.
❌ Loss of bracing
❌ Lack of pelvic control/optimal position

Let’s talk about bracing and why this is important. Bracing, especially with heavy squatting, provides stability to the spinal column. Bracing also provides stability to the pelvic region, which ends up influencing hip position. If you lose your brace at the bottom of the squat, it can affect the femoral head position relative to the pelvis and potentially cause these symptoms. So follow along in the video to learn a simple tip to work on bracing with sit squats.

What about pelvic control and pelvis position? There is a lot of debate about butt winking with squatting and we have covered this topic in other posts/articles. Personally, I only focus on this when someone is having symptoms in the pelvic region with squatting, otherwise, I don’t get too caught up with it. Ultimately, you need to be able to control movement in this area. Follow along with this video to work on body awareness and control of this region with sit squats.

If your hip hurts while you’re trying to sleep on your side, physical therapist Christine Koth explains what could be causing hip pain at night and how to adjust your position to get some sleep.
If you have outer hip pain, such as hip bursitis, you may feel pain either on the hip you’re laying on, or the hip you’re not laying on.
If you have pain in the hip you’re laying on, pain caused by direct pressure on the joint, then pay attention to your mattress surface. You may find relief by adding a memory foam pad or mattress topper to your mattress to help distribute your weight more evenly and provide extra cushion for that hip joint.
If you have pain on the hip you’re not laying on, try sleeping with a pillow between your knees and ankles. It may take some time to find a position that works with your pillow, and you may even have to try out pillows of different densities. The goal is to have your legs rest parallel to each other, so your knee is not lower than your hip. This can be accomplished by using a fat pillow, resting lengthwise from knees to ankles.
Sleeping with a pillow between your knees and ankles is a good way to relieve SI joint pain while sleeping on your side, as it reduces the strain on muscles attaching to the top of the thigh bone (greater trochanter). This strain on the hip is directly related to tight hip flexors (specifically a tight iliacus muscle), which causes an anterior pelvic rotation, and pulls on the lower back and hip joint.
Whatever the pillow position you choose, the main purpose of sleeping with the pillow is to alleviate the pull on your hip’s soft tissues and keep your pelvis in a natural alignment.
When you allow your leg to rest naturally while lying down, there is a slight dip that enables your legs to connect and rest. When this happens, your leg’s natural pull adds more pressure to the muscles around your hip joint, which can then cause hip pain when sleeping.

Patient has been experiencing gas pain for the past 30 years. The pain has gotten worse in the past 7-8 years where it would wake him up in the middle of the night every 2-3 hours.
Meticulous structural test and Gonstead examination reveals structural abnormalities at the 5th lumbar and 6th dorsal vertebrae. The 5th Lumbar nerve obstruction resulted in constipation that led to increase bowel gas. The 6th Dorsal nerve obstruction resulted in his pyloric sphincter to be wide open when it’s suppose to be closed.
